Admission Checklist

Checklist of Items Needed for Admission

In order to help us maintain accurate and up-to-date information for each child in attendance at Little People, we are required by our Regulatory Agencies to ask each parent to provide us with the following information:

• Application Card

• Medical Examination

• Vision & Hearing Screening

• Each Parent's Social Security No.

• Complete Up-to Date Shot Record

• Fee Agreements

• Emergency Card

• DPE Income Questionnaire

• Child's Social Security No.

• Other

IT IS MOST IMPORTANT THAT WE RECEIVE ALL THE NECESSARY FORMS, since they are required by the City of Allentown & the State of Pennsylvania.
